Emailing Your Requisition to Alpha Laboratories
Alpha Laboratories accepts electronic requisitions by email for patients visiting our Patient Service Centres without a print out of the requisitions. Please email a PDF copy of your requisition to our dedicated email address – mailmyREQON@alphalabs.ca
Important Guidelines:
- Send one requisition per email
- Use the patient’s full name as the email subject line
- Double-check that all patient details are accurate
- Ensure the requisition is clear and legible
