Please complete the Booking request form to arrange an appointment. You will need to submit a separate booking request for each exam type (i.e., Ultrasound or BMD). The receptionist will email or call you to confirm your appointment details.

Alpha Diagnostic Imaging accepts all requisitions from any diagnostic imaging providers. X-rays are provided on a walk-in basis and do not require an appointment.
