What to include:

  1. Your full name
  2. Location of clinic (Don Mills or Lawrence)
  3. Date on which you attended facility.
  4. Modality (X-Ray, BMD, or Ultrasound): List services that you had or were scheduled for.
  5. Complaint in detail

Every complaint will be diligently investigated. We are committed to providing timely resolutions and you will be promptly notified of the progress and outcome of the process within a 2 week’s time.
